Enabling Patients to Manage Their Own Health Records: A New Paradigm, Marianne Hudgins, MD, Consultant and Primary Care Physician

Even with the availability of electronic health records, patient data remains fragmented, particularly for providers caring for patients who receive services outside their specific electronic health system. A lack of access to relevant information leads to delays, duplication of services, and frustration for all parties involved.

This session will propose a new paradigm. What if certified copies of all of a patient’s health records were held in a singular, secure vault, under the control of the individual or their guardian? What if providers could access this source to view information regardless of where healthcare was received?

Marianne Hudgins, MD, practiced primary care medicine for eighteen years at Kansas City Internal Medicine before transitioning into an administrative role as Quality Director at an independent family practice clinic. Throughout her career, she served as the physician leader, guiding her practices through the shift to electronic medical records and later through successful participation in various quality improvement initiatives, including earning Patient-Centered Medical Home recognition. She also led each practice’s involvement in several CMS pilot programs aimed at improving patient outcomes and reducing costs. Additionally, she has served as a consultant to companies and organizations that provide resources directly impacting quality patient care.

Street Medicine, Jaynell "KK" Assman, APRN-C, Family Nurse Practitioner; Founder and CEO, Care Beyond the Boulevard, Kansas City, KS

Street medicine is the practice of delivering healthcare services directly to individuals experiencing homelessness or living in unstable housing. Essential medical care is provided in locations where these individuals reside, such as street, parks or shelters. This approach is crucial as those experiencing homelessness typically face significant health disparities and have limited access to Provent ice and primary healthcare services. 

Care Beyond the Boulevard Inc. is a street medicine provider in the Kansa City metro area. Their mission is to provide high quality medical and mental health care and support to the homeless and underserved populations of Kansas City outside the bounds of traditional healthcare settings, and in so doing become a force for health care equality and social change.

ABIM: Successful completion of this CME activity enables the participant to earn up to 18 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.
